Please bring any pertinent medical records to your office visit such as: your driver's license or other photo ID, your current insurance card, and bring all prescription medications you are currently taking. Please arrive early to your first appointment to complete registration paperwork.


YOUR FIRST APPOINTMENT

WHAT TO DO:

  • Contact your insurance company to determine if our services are covered and if you need a referral from your primary care provider. Please get this referral prior to your first visit.
  • Please arrive 20 minutes before your scheduled appointment time to complete information forms.
  • Please bring all insurance card(s), emergency contact numbers and a bag containing all of your current prescription medications. Be prepared to inform our medical staff of any known allergies.
  • If you do not have insurance, we are happy to serve you, however, we DO require payment for services rendered. Bring $100 cash to your appointment; you will be billed for any additional balance.
  • This is a very busy medical practice. Out of courtesy to others needing an appointment, please notify our office at least 24 hours before your scheduled appointment if you will be unable to keep it.
  • If your appointment is for a vasectomy consultation and you decide to proceed with the procedure, a deposit of $150 (cash or credit card) will be required in order to schedule it. The deposit will be refunded to you (less any outstanding account balance) the first week of the month following the procedure. The deposit will be forfeited if you do not show up, or if you do not cancel the appointment a full week prior to the scheduled procedure date.
