Financial Policy

East Alabama Urology Associates (EAUA) is committed to providing excellent urological healthcare. For these services, we require prompt payment. EAUA participates with many insurance plans. If your insurance plan does not cover our services or if we are not a contracted provider with your insurance carrier, payment is expected in full at the time of your visit. Please be sure your insurance information is current at the time of service. It is your responsibility to provide accurate insurance information to us.

We will file your insurance and allow 45 days for your insurance company to make payment on your claim. If your insurance company fails to render payment, you are still responsible to pay in full. If your insurance company requires a referral from your primary care physician, it will be your responsibility to obtain the referral prior to your visit. If you do not have a valid referral, your appointment will be rescheduled.

You will be asked to present your insurance card(s) at every visit. Co-pays are required by most insurance plans and they will be collected prior to receiving medical services. If you do not have your copay, your appointment may be rescheduled.

If you do not have insurance, you are responsible for the bill. Payment arrangements will be required prior to receiving medical services. We suggest bringing $100.00 cash to each appointment.

EAUA accepts cash, checks, VISA, MasterCard and Discover. Patient accounts are billed at the end of each month. If you have questions about your bill when you receive it, please call our office to have your questions answered. Please do not ignore any bills sent to you from EAUA because after an account has been billed twice and no payment is received, the account is sent to collections.
